Job summary

CarePartners in Ontario is seeking Part-Time RNs and RPNs for flexible home-care roles in the Goderich area. You will develop and maintain care plans, work with patients in-person and virtually, and provide nursing services across settings such as in-home, clinics, retirement homes or schools.

The role offers a competitive wage grid with progression by hours and role, mileage reimbursement at $0.48 per km, supportive team culture, ongoing education, and scheduling flexibility, including weekends

Qualifications

• Must be a Registered Nurse (RN) or Registered Practical Nurse (RPN) with current CNO registration.
• Must hold a valid driver’s license and access to a vehicle for regular travel.
• English language proficiency, both verbal and written.

Responsibilities

• Develop and maintain an effective Care Plan for Community Health Services.
• Provide nursing services to patients in-person and virtually.
• Deliver services in various settings: in-home, clinic, retirement home or school.
• Communicate with patients, families, and the care team.
• Perform other clinical duties as required.

This is a Part-Time position in the Goderich ON, area.

Hours of work are 8 am - 8 pm within a 2-week rotation inclusive of weekends, weekdays, and on-call work.

Wages

• For Registered Nurses (RN)- This position offers a competitive compensation grid, with progression based on hours worked and role-specific criteria. The RN per general visit range is $ 27.81 to $41.07 and the RN hourly range is $32.08 to $47.38. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.
• For Registered Practical Nurses (RPN)- This position offers a competitive compensation grid, with progression based on hours worked and role-specific criteria. The RPN per general visit range is $ 23.66 to $29.48 and the RPN hourly range is $27.17 to $33.37. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.

What The Role Involves

• Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
• Working with patients (both in-person and virtually) to provide exceptional nursing services
• Providing services in a variety of settings which may include in-home, clinic, retirement home or school
• Communicating and liaising with patients, family members and others within the interdisciplinary care team
• Performing other clinical duties as required

What You Bring

• Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
• Current College of Nurses of Ontario (CNO) registration in good standing
• A strong desire and commitment to making a difference in the lives of our patients
• Exceptional interpersonal skills
• A valid driver’s license and vehicle as regular travel is required
• Excellent English verbal and written communication skills
• CPR Certification
• Current immunization record including 2 step TB test
• To protect our patients, we require a current vulnerable sector check and a clear background check
