Job summary

CarePartners, a leading home health care provider in Ontario, is looking for Registered Nurses (RNs) and Registered Practical Nurses (RPNs) in the Burk's Falls area. Qualifications

  • Current College of Nurses of Ontario (CNO) registration in good standing.
  • A valid driver’s license and vehicle as regular travel is required.
  • Current immunization record including 2 step TB test.

Responsibilities

  • Develop and maintain an effective Care Plan with respect to Community Health Services.
  • Provide exceptional nursing services to patients both in-person and virtually.
  • Communicate and liaise with patients and family members within the care team.

This is a Full-Time/Part-Time/Casual position in the Burk's Falls, Sundridge area.

Hours of work are within a 2-week rotation inclusive of weekends, weekdays, and on-call work.

Wages

  • For Registered Nurses (RN)- This position offers a competitive compensation grid, the hourly range is $29.90 to $44.16 and the general visit rate is $29.32 to $43.28.
  • For Registered Practical Nurses (RPN) - This position offers a competitive compensation grid, the hourly range is $23.37 to $28.74, and the general visit rate is $23.37 to $28.74.
What The Role Involves
  • Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
  • Working with patients (both in-person and virtually) to provide exceptional nursing services
  • Providing services in a variety of settings which may include in-home, clinic, retirement home or school
  • Communicating and liaising with patients, family members and others within the interdisciplinary care team
  • Performing other clinical duties as required
What You Bring
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • A strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills
  • A valid driver’s license and vehicle as regular travel is required
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record including 2 step TB test
  • To protect our patients, we require a current vulnerable sector check and a clear background check
