The Terraces Assisted Living provides primary care to the Terraces residents who reside in individual suites within the building. In this role you will support the assessment and development of the resident care plans. You will work collaboratively with the multidisciplinary team and families to support resident transitions in care as well as health centre coverage as required.
Baycrest Terraces is also connected to The Kimel Family Centre for Brain Health and Wellness, which provide research-based activities to promote brain health and aging using a wide range of recreational, fitness, social and educational programs.
