Job summary

Baycrest Hospital is seeking a Registered Practical Nurse (RPN) for the Complex Continuing Care Unit (6E) to provide specialized post-acute inpatient care to older adults with complex medical conditions. This permanent part-time role offers a night shift schedule, with a bi-weekly commitment of 30 hours.

You will collaborate with the interdisciplinary team, plan and evaluate nursing interventions, and support quality-of-life goals within a non-acute hospital setting.

Qualifications

• Current registration in good standing with the College of Nurses of Ontario (CNO).
• Current Basic Cardiac Life Support (BCLS) Level C with AED is required.
• Experience caring for geriatric patients with complex medical care needs preferred.
• Psychogeriatric experience, i.e. mental health assessment and intervention, preferred.
• Excellent interpersonal, communication and problem-solving skills.
• Basic computer skills – Meditech, Microsoft Word, and Excel preferred.

Responsibilities

• Plans for, delivers and evaluates nursing interventions within the RPN scope of practice.
• Provides client-centred care within interdisciplinary care team to meet personal care, activation, recreation and spiritual needs.
• Collaborates with the Registered Nurse to develop and revise the individualized care plan.

The Complex Continuing Care Program helps patients and their families identify strategies that support quality-of-life goals as well as short term and long term care needs.

Key Responsibilities

• Plans for, delivers and evaluates nursing interventions, including medications, as well as treatments which fall within the RPN’s scope of practice
• Provides client-centred care within interdisciplinary care team to meet the personal care, activation, recreation and spiritual needs of clients
• Collaborates with the Registered Nurse to develop and revise the individualized care plan
