You will provide health navigation, nursing clinics and chronic disease management for senior clients, living independently in the community and within Toronto Senior Housing Buildings, who can benefit from an interprofessional team approach to address physical, cognitive, psychosocial, social and environmental issues that are having an impact on function, with the focus on chronic disease management, frailty, mental health and primary care. You will provide health education, wellness checks to help clients achieve their health goals to improve their quality of life. Your assessment and goal setting will inform the comprehensive assessment and treatment plan of clients. In this role you will be working closely with primary care physicians to support attachment to primary care and collaborative in care.
