Job summary

Baycrest Hospital is seeking a Registered Practical Nurse (RPN) for the Complex Continuing Care Unit (6E) on a permanent part-time basis. The role involves planning, delivering and evaluating nursing interventions, in collaboration with the care team, to support medically complex older adults.

You will provide patient-centred care, contribute to care plan development, and address personal, activation, recreation and spiritual needs within a non-acute setting.

Qualifications

• Current registration, in good standing, with the College of Nurses of Ontario (CNO).
• Current Basic Cardiac Life Support (BCLS) Level C with AED.
• Experience in geriatrics with complex medical needs preferred.
• Psychogeriatric experience and handling responsive behaviours preferred.
• Excellent interpersonal, communication and problem solving skills.
• Basic computer skills with Meditech, Word and Excel preferred.

Responsibilities

• Plans, delivers and evaluates nursing interventions and treatments within the RPN scope.
• Provides client-centred care within an interdisciplinary team to meet personal, activation, recreation and spiritual needs.
• Collaborates with the Registered Nurse to develop and revise the individualized care plan.

Job Summary

The Complex Continuing Care Unit provides care for older patients with complex medical conditions in a non-acute hospital setting. The Complex Continuing Care Program helps patients and their families identify strategies that support quality-of-life goals as well as short term and long term care needs.

Key Responsibilities

• Plans for, delivers and evaluates nursing interventions, including medications, as well as treatments which fall within the RPN’s scope of practice
• Provides client-centred care within interdisciplinary care team to meet the personal care, activation, recreation and spiritual needs of clients
• Collaborates with the Registered Nurse to develop and revise the individualized care plan

Qualifications

• Current registration, in good standing, from the College of Nurses of Ontario (‘CNO’) is required
• Current Basic Cardiac Life Support (‘BCLS’) certificate, Level C - Automated External Device (‘AED’) is required
• Experience in caring for geriatrics patients with complex medical care needs preferred
• Psychogeriatric experience, i.e. mental health assessment and intervention and working with responsive behaviours preferred
• Excellent interpersonal, communication and problem solving skills
• Basic computer skills – Meditech, Microsoft Word, and Excel preferred
