Job summary

Baycrest Hospital in Toronto is seeking a Registered Practical Nurse (RPN) for Apotex 4 on a permanent part-time basis. The role supports cognitive care units, with day shifts and occasional weekend rotation, under the supervision of RNs.

You will provide patient-centred nursing care, participate in developing and implementing care plans, and use MDS/RAI and clinical skills. Must hold CNO registration and BCLS Level C with AED; LTC experience preferred.

Qualifications

• Current registration, in good standing, from the College of Nurses of Ontario is required.
• Current Basic Cardiac Life Support, Level C (AED) is required.
• Experience in Long-Term Care or community setting preferred.
• Psychogeriatric experience and dementia care preferred.
• Certificate in Dementia Care preferred.
• Training in Gentle Persuasive Approach and U-First preferred.
• Excellent interpersonal, communication and problem solving skills.
• Excellent verbal and written communication skills.
• Basic computer skills – MDS/RAI, Point Click Care, Word and Excel preferred.

Responsibilities

• In collaboration with RN provides sound clinical knowledge and skills, acts as clinical expert and resource and provides comprehensive, client/family centred health care services from admission, through transfers and discharge.
• Plans for, delivers and evaluates nursing interventions, including medications and treatments within the RPN scope of practice.
• Provides client-centred care within interdisciplinary care team to meet personal care, activation, recreation and spiritual needs of clients.
• Performs client assessments.
• Collaborates with the Registered Nurse to develop and revise the individualized care plan.

Job Summary

Apotex 4 is a Cognitive Support Unit. Residents residing on this floor are living with a wide range of cognitive and physical changes to their abilities – from mild to severe cognitive impairment and from ambulating independently, to being wheel-chair bound.

Key Responsibilities

• In collaboration with RN provides sound clinical knowledge and skills, acts as clinical expert and resource and provides comprehensive, quality client/family centred health care services from admission, through transfers and discharge
• Plans for, delivers and evaluates nursing interventions, including medications, as well as treatments which fall within the RPN’s scope of practice
• Provides client-centred care within interdisciplinary care team to meet the personal care, activation, recreation and spiritual needs of clients
• Performs client assessments
• Collaborates with the Registered Nurse to develop and revise the individualized care plan

Qualifications

• Current registration, in good standing, from the College of Nurses of Ontario (‘CNO’) is required
• Current Basic Cardiac Life Support (‘BCLS’) certificate, Level C - Automated External Device (‘AED’) is required
• Experience in Long-Term Care, clients in day programs or residents in a Long-Term Care and Community Setting preferred
• Psychogeriatric experience, i.e. mental health assessment and intervention and working with responsive behaviours including various types of dementia, delirium, mental health issues preferred
• Certificate in Dementia Care preferred
• Training in Gentle Persuasive Approach and U-First preferred
• Excellent interpersonal, communication and problem solving skills
• Excellent verbal and written communication skills
• Basic computer skills – including MDS/RAI, Point Click Care, Microsoft Word and Excel preferred
