Job summary

Prima Living in Whitby, Ontario, is seeking a Registered Practical Nurse (RPN) to provide compassionate nursing care in a long-term care setting. The role includes administering medications, assessing residents, and coordinating with the care team.

The position is full-time, onsite, with days, evenings, and weekends rotating shifts. Requires Practical Nursing diploma, good standing with CNO, CPR Level C/HCP, and a criminal background check.

Qualifications

• Diploma from a recognized Practical Nursing program.
• Current registration in good standing with the College of Nurses of Ontario (CNO).
• Current CPR Level C or Healthcare Provider certification; First Aid may also be required.
• Up-to-date immunizations in accordance with applicable public health requirements.
• Successful completion of a criminal background check, including Vulnerable Sector Screening.

Responsibilities

• Deliver nursing care in accordance with established care plans, clinical standards, and residents' individual needs.
• Administer medications and treatments as prescribed and monitor residents' responses.
• Assess residents, identify changes in health status, and promptly report concerns to the appropriate healthcare professionals.
• Maintain accurate and timely clinical documentation in accordance with professional standards and CNO guidelines.
• Promote residents’ comfort, dignity, independence, and overall well-being.
• Assist with activities of daily living when required and ensure residents receive compassionate, person-centred care.
• Provide emotional support and communicate effectively with residents and their families.
• Collaborate with the interdisciplinary healthcare team to develop, implement, and evaluate individualized care plans.
• Communicate changes in residents’ conditions and contribute to effective care coordination.
• Support a positive, respectful, and collaborative team environment.
• Follow organizational policies, procedures, and applicable provincial healthcare regulations.
• Maintain infection prevention and control practices and promote a safe environment for residents, visitors, and team members.
• Protect residents’ privacy and confidentiality while upholding professional nursing standards.
