Belcrest Nursing Homes in Ontario is seeking a Registered Practical Nurse (RPN) to provide direct nursing care to residents in accordance with the CNO standards and Fixing Long-Term Care Act 2021.
You will perform assessments, administer medications, support care planning, wound and palliative care, and supervise PSWs, while collaborating with RNs and physicians to ensure safe, compassionate, high-quality resident care.

Reports To: Director of Care / RN Supervisor
The Registered Practical Nurse (RPN) provides direct nursing care to residents in accordance with the standards of practice set by the College of Nurses of Ontario (CNO), the Fixing Long-Term Care Act, 2021, and facility policies. The RPN ensures safe, competent, and compassionate care to residents while collaborating with the interdisciplinary healthcare team.
Administer medications and treatments as prescribed
Perform nursing assessments within RPN scope of practice
Monitor and document residents’ health status and report changes
Provide wound care, palliative care, and chronic disease management
Assist with activities of daily living (ADLs) as needed
Participate in developing, implementing, and evaluating individualized care plans
Update care plans based on resident condition and physician orders
Attend care conferences with families and healthcare team
Maintain accurate and timely documentation in electronic or paper health records
Complete incident reports and required regulatory documentation
Supervise and provide direction to Personal Support Workers (PSWs)
Collaborate with Registered Nurses (RNs), physicians, physiotherapists, and other team members
Act as a charge nurse when required
Follow infection prevention and control (IPAC) protocols
Adhere to workplace health and safety standards
Participate in quality improvement initiatives
Diploma in Practical Nursing from an accredited program
Current registration and good standing with the College of Nurses of Ontario
Current CPR and First Aid certification
Knowledge of gerontology and long-term care standards
Experience in long‑term care (preferred)
Strong clinical assessment skills
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Compassionate, patient‑centered approach
Strong organizational and time management skills
Knowledge of electronic health record systems
Physically demanding (lifting, transferring residents)
Fast‑paced environment with multiple priorities
