REGISTERED NURSE – SUBSTANCE USE & MENTAL HEALTH

Full-Time, Regular Positions

Salary Range: $71,709 - $84,364 (Includes health benefits and HOOPP pension plan)

South Riverdale Community Health Centre is a non-profit organization providing primary healthcare, social, and community outreach services in East Toronto, focusing on health promotion and disease prevention. Our mission is to improve the lives of those facing barriers to well-being by engaging communities and ensuring equitable access to quality care through evidence-informed programs.

These positions are part of the new HART Hub initiative, led by East Toronto Health Partners Ontario Health Team, involving multiple community organizations and health services.

The Registered Nurse – SU&MH provides nursing support within the department, including screening, assessments, primary care, wound care, system navigation, health education, and overdose response. The role involves collaboration with interdisciplinary teams, some outreach, and onsite work, with flexible hours including evenings and weekends.

Responsibilities:

  • Support a welcoming, non-judgmental, and safe environment
  • Conduct intake assessments, health histories, psychosocial evaluations, and physical assessments to establish care plans and connect clients with services
  • Manage episodic and chronic health conditions in primary care
  • Provide supportive counseling, health education, and referrals
  • Offer preventative and crisis counseling as needed
  • Assess the need for prescriptions, tests, and diagnostics within RN scope
  • Dispense medications as delegated and authorized
  • Develop care plans with clients
  • Work within an interdisciplinary team, following medical directives and consulting colleagues
  • Respond to emergencies, including overdoses
  • Build therapeutic relationships while maintaining boundaries
  • Participate in team meetings, case conferences, and community outreach
  • Lead health education workshops and provide harm reduction information
  • Implement health promotion strategies considering social determinants of health
  • De-escalate conflicts and document care accurately
  • Support research and data collection activities
  • Identify health trends for service planning and advocacy
  • Assist with inventory management
  • Maintain confidentiality and collaborate across teams

Qualifications:

  • BSc in Nursing from a recognized university
  • Current registration with the College of Nurses of Ontario
  • Wound Care certification (asset)
  • 3-5 years experience in community, hospital, or public health settings
  • Thorough nursing knowledge, including health histories, assessments, and management plans
  • Experience in harm reduction and working with diverse populations, including Indigenous peoples, racialized groups, women in sex work, and those with mental health or trauma histories (asset)
  • Knowledge of community resources and strong advocacy skills
  • Experience with complex wound care, infection control, emergency response, overdose management, oxygen, and Naloxone
  • Physical ability to support overdose situations
  • Skills in conflict de-escalation, boundary setting, and cultural competency
  • Proficiency with computer applications, strong communication skills, and active listening
  • Willingness to learn, non-judgmental attitude, and ability to work collaboratively
  • Police clearance and proof of COVID-19 vaccination required
