South Riverdale Community Health Centre is a non-profit, multi-service organization providing primary healthcare, social, and community outreach services with an emphasis on health promotion and disease prevention, primarily serving East Toronto. These positions are part of the new HART Hub, an initiative of East Toronto Health Partners Ontario Health Team (ETHP OHT), led by South Riverdale Community Health Centre and St. Michael’s Homes, with support from various community organizations and health services.
The Registered Nurse – SU & MH provides nursing support across teams within the SU & MH department. Responsibilities include conducting screening and intake assessments, providing primary and wound care, system navigation, health education, and overdose response. The role involves collaboration with interdisciplinary teams, partners, and service users, and requires working some flexible hours, including evenings and weekends, with onsite presence.
