POSITION: Registered Nurse – Substance Use & Mental Health

POSITION TYPE: Permanent

COMMUNITY HEALTH CENTRE: South Riverdale Community Health Centre (ON)

LOCATION: Toronto, ON

POSITION DESCRIPTION:

FULL TIME, REGULAR POSITIONS

SALARY RANGE: $71,709 - $84,364

Includes health benefits and HOOPP (Healthcare of Ontario Pension Plan)

South Riverdale Community Health Centre is a non-profit, multi-service organization providing primary healthcare, social, and community outreach services with an emphasis on health promotion and disease prevention, primarily serving East Toronto. Our mission is to improve the lives of people facing barriers to physical, mental, spiritual, and social well-being. We engage our clients and communities meaningfully, ensuring equitable access to primary health care and delivering quality, evidence-informed programs and services. Our community services include primary health services, health promotion, harm reduction, environmental health, a community food centre, and population-based programs for marginalized populations. We value health equity, inclusion, and respect in our work and service delivery.

These positions are part of the new HART Hub, an initiative of East Toronto Health Partners Ontario Health Team (ETHP OHT), led by South Riverdale Community Health Centre and St. Michael’s Homes, with support from various community organizations and health services.

The Registered Nurse – SU & MH provides nursing support across teams within the SU & MH department. Responsibilities include conducting screening and intake assessments, providing primary and wound care, system navigation, health education, and overdose response. The role involves collaboration with interdisciplinary teams, partners, and service users, and requires working some flexible hours, including evenings and weekends, with onsite presence.

Responsibilities include:

  • Supporting a welcoming, non-judgmental, and safe environment
  • Completing intake and assessments (health histories, psychosocial, functional, family, physical, wound assessments) to establish care plans and connect clients to services
  • Managing episodic and chronic conditions in primary health care
  • Providing supportive counseling, education, and referrals
  • Assessing the need for prescriptions, labs, and diagnostic tests within RN scope
  • Dispensing medication as delegated and directed by physicians and nurse practitioners
  • Working with clients to identify health issues and develop care plans
  • Collaborating within interdisciplinary teams and responding to medical emergencies, including overdose events
  • Building rapport and maintaining therapeutic relationships
  • Participating in team meetings, case conferences, and off-site clinical activities
  • Facilitating health education workshops and harm reduction activities
  • Incorporating health promotion strategies, recognizing social determinants of health and structural inequities
  • De-escalating conflicts and maintaining documentation per protocols
  • Supporting research and evaluation activities, collecting and analyzing data
  • Using health data to inform service planning and advocacy
