Job summary

VON Canada is seeking a Registered Nurse for visiting home-based care across children, youth, adults and seniors. You’ll perform holistic assessments, deliver primary care in clients’ homes, clinics, or virtually, and collaborate with the healthcare team.

Qualifications

• Must be licensed as a Registered Nurse in the province of practice.
• Community health experience is an asset.
• Must enjoy driving, have a personal vehicle and possess a current driver’s license and proof of vehicle insurance.
• A satisfactory Criminal Records Check is required; a Vulnerable Sector Search and/or Child Abuse Registry Check may also be necessary.
• Must be able to wear Personal Protective Equipment (PPE).

Responsibilities

• Client assessments to develop individual care plans.
• Provide primary care in collaboration with other healthcare professionals.
• Wound care, including complex and negative pressure wounds.
• Chronic disease management, e.g. diabetes care and education.
• IV therapy and CVAD management.
• Phlebotomy and specimen collection.
• Dialysis management.
• Rehabilitative programs.
• Palliative and end of life care.
• Medication management.
• And many other critical nursing skills.
