Job Overview:

The primary role of the Registered Nurse at the Gage Transition to Independent Living (GTIL) is to assess clients' health and ability to manage their health and related activities of daily living in the community. The Registered Nurse will evaluate and implement individualized client education in preparation for independent living. The Registered Nurse will also facilitate health education and orientation for Personal Care Attendants based on the client's needs.

Accountabilities:

Admission/Discharge:

  1. To work in collaboration with Intake Coordinator to assess, determine eligibility, and admit new clients into the program.
  2. To participate as a member of the Admissions Committee to determine acceptance of clients into the program and determine any program extensions (as needed).
  3. To liaise with the Physician appointed to the GTIL regarding the health needs of clients upon admission and discharge, and for consultation when clients have a change in medical status.

Health Management:

  1. To participate in the ongoing assessment of the health needs and goals of the clients.
  2. To discuss the client's choice of Family Physician and ensure each client is connected to a Family Physician. With consent from the client, liaise with the Physician as appropriate.
  3. To manage a change in client's medical status resulting in unpredictability of outcomes and care requirements, including: assessing the risk of changes to the client's health condition in response to treatment and their ability to manage this within the program.
  4. To facilitate learning which pertains to clients' knowledge and understanding of their health and bodily functions, and the implications of how these relate to their physical disability (i.e. bowel function, skin care, bladder function, etc.).
  5. To provide individual counseling re: health issues, needs, and choices.
  6. To facilitate learning that prepares clients to access services in the community to maintain good health, including: ensuring clients are aware of and able to use community services (i.e. medical and nursing services).
  7. To ensure clients create a safe environment for themselves and that they are prepared to deal with emergencies.
  8. To facilitate learning in friendship, intimacy, human sexuality, and reproductive health including the discussion with individual clients about sexual adjustment and integration, as identified by the client.

Education:

  1. To assist with the onboarding, orientation, teaching, and supervising/monitoring of ongoing competency of unregulated care providers (i.e. Personal Care Attendants) in accordance with the Regulated Health Professions Act and CNO Practice Guidelines 2013.
  2. To act as a resource for Personal Care Attendants and clients when care routines are modified or clients' choices result in alterations in practice.
  3. To facilitate or prepare and present ongoing in-service education programs as needed for Personal Care Attendants.
  4. To provide leadership in designing and delivering education arising from new or changing clinical practices.
  5. To keep abreast of current trends and resources in the areas of best practices, transitional living, issues around disability, sexuality, health management, and health care supplies.
  6. To support, mentor, and coach Personal Care Attendants and nursing students (as appropriate), in achieving their learning goals and objectives.

Consultation/Collaboration:

  1. To be aware of conditions and measures that may affect the safety of clients, students, employees, physicians and visitors, including as required: monthly safety audits, participation in the West Park Joint Occupational Health and Safety Committee (JOHSC).
  2. To be aware of best practices and contribute to meaningful discussions as an active member within the Nursing Professional Standards and Issues Committee (NPSIC).
  3. To collaborate with Infection Prevention and Control to ensure best practices are being implemented at a community level.
  4. To collaborate with the Clinical Nurse Educator to facilitate educational and training sessions for staff, and to act as a resource for the team when the GTIL Registered Nurse is away, as required.
  5. To collaborate with and support Clinical Service Manager with recruitment, and contribute to the evaluation of staff when appropriate.

Key Qualifications:

  1. Current registration with the College of Nurses of Ontario and in good standing.
  2. Baccalaureate degree in Nursing from an approved nursing program preferred.
  3. Physical assessment skills.
  4. Clinical experience in community practice and BLS preferred.
  5. Minimum 2 years clinical experience and knowledge working with people with disabilities in areas of health in the community.
  6. Ability to perform the essential clinical duties of the job (i.e. care and management related to tracheostomy, ventilator, suctioning, intermittent catheterization, bowel routines, skin and wound).
  7. Skills in safe body mechanics, lifts, and transfers.
  8. Leadership skills including effective management of challenging interpersonal relations.
  9. Excellent communication skills (verbal and written) and the ability to develop and maintain strong working relationships centered on mutual respect between clients, colleagues, and other professionals and community partners.
  10. Personal and professional characteristics necessary for effective functioning in a satellite location including: sound judgment, flexibility, critical thinking, problem solving, initiative, and follow through on tasks and projects, and the ability to work independently.
  11. Experience and skill in adult education related to health issues (experiential or credential).
  12. A strong understanding and appreciation of the psychosocial aspects of living with a disability and the barriers and challenges faced by people with disabilities in getting their needs met.
  13. Demonstrated commitment to continuing professional development.
  14. Clinical experience with health promotion and education/teaching preferred.
  15. Clinical experience related to community mental health preferred.
