Job summary

A leading home healthcare agency is seeking a Registered Nurse to ensure high standards of documentation and quality of care. The role involves reviewing OASIS documents, conducting clinical record reviews, and maintaining compliance with agency standards. Ideal candidates will possess strong organizational and interpersonal skills, along with a valid RN license and relevant home health experience.

Qualifications

  • Experience in QA of a home care agency and OASIS experience.

Job description

Qualifications
  • Registered Nurse (RN) with a current MA license.
  • Minimum of two (2) years of home health experience, including experience in QA of a home care agency and OASIS experience.
Knowledge and Skills
  • Working knowledge of Medicare regulations, State regulations, and home health documentation requirements.
  • Good organizational, interpersonal, and judgment skills.
  • Ability to develop action plans and resolve identified inconsistencies.
Responsibilities
  1. Review and process OASIS documents (Admissions, Recertifications, Resumptions, Transfers, and Discharges).
  2. Review nurses' notes and other related documents.
  3. Monitor documentation trends through chart reviews; maintain high documentation standards.
  4. Conduct Clinical Record Reviews (CRR) focusing on documentation verification, quality of care, visit utilization, adherence to the plan of care and policies, and inter-disciplinary communication to ensure proper documentation for reimbursement.
  5. Maintain agency CRR compliance at 80% or greater.
  6. Communicate effectively with all levels of agency staff.
  7. Report trends and issues to the Clinical Manager/Administrator, including process changes or staff training needs based on QA data.
  8. Participate in agency orientation and in-service training programs.
  9. Prepare QA meeting notifications and compile reports from other agency committees for QA program integration.
  10. Assist in clinical training programs and development of training materials.
  11. Participate in the agency's emergency preparedness plan.
  12. Perform other duties as assigned.
