Job summary
A leading home healthcare agency is seeking a Registered Nurse to ensure high standards of documentation and quality of care. Qualifications
- Minimum of two (2) years of home health experience.
- Experience in QA of a home care agency and OASIS experience.
Responsibilities
- Review and process OASIS documents (Admissions, Recertifications, etc.).
- Monitor documentation trends through chart reviews.
- Conduct Clinical Record Reviews (CRR) focusing on documentation verification.

Qualifications
- Registered Nurse (RN) with a current MA license.
- Minimum of two (2) years of home health experience, including experience in QA of a home care agency and OASIS experience.
Knowledge and Skills
- Working knowledge of Medicare regulations, State regulations, and home health documentation requirements.
- Good organizational, interpersonal, and judgment skills.
- Ability to develop action plans and resolve identified inconsistencies.
Responsibilities
- Review and process OASIS documents (Admissions, Recertifications, Resumptions, Transfers, and Discharges).
- Review nurses' notes and other related documents.
- Monitor documentation trends through chart reviews; maintain high documentation standards.
- Conduct Clinical Record Reviews (CRR) focusing on documentation verification, quality of care, visit utilization, adherence to the plan of care and policies, and inter-disciplinary communication to ensure proper documentation for reimbursement.
- Maintain agency CRR compliance at 80% or greater.
- Communicate effectively with all levels of agency staff.
- Report trends and issues to the Clinical Manager/Administrator, including process changes or staff training needs based on QA data.
- Participate in agency orientation and in-service training programs.
- Prepare QA meeting notifications and compile reports from other agency committees for QA program integration.
- Assist in clinical training programs and development of training materials.
- Participate in the agency's emergency preparedness plan.
- Perform other duties as assigned.
