Job summary

ParaMed in Muskoka, Ontario is seeking a Registered Nurse to visit patients in their homes, assess needs, and manage care plans. You will travel within the designated area, use assessment tools, and maintain accurate patient records to support comfort and independence at home.

As part of the team, you’ll engage patients through rapport, collaborate with families, and participate in on-call coverage to ensure safety and timely care, training and support are provided throughout.

Qualifications

  • Registration with the CNO as a Registered Nurse in good standing.
  • Valid driver's license and access to a vehicle.
  • Willingness to travel to patient residences within a designated area.
  • Experience in delivering community care is an asset.

Responsibilities

  • Using appropriate assessment tools to assess patients’ individualized needs.
  • Executing and monitoring care plans and updating records.
  • Engaging patients with a collaborative approach to build rapport and trust.
  • Maintaining and updating professional patient records related to encounters and plans.
  • Participating in on-call coverage to ensure patient safety and care needs after hours.
