Job summary

ParaMed in Ontario is seeking a Registered Nurse to perform scheduled home visits, assessing patients and adapting plans to meet individual needs. You will maintain records, collaborate with families and care teams, and support patients to live safely at home.

The role requires travel within assigned areas, on-call coverage and a commitment to compassionate, high-quality community care. Competitive pay and benefits are provided, with both part-time and full-time opportunities.

Qualifications

• Registered Nurse with active CNO registration in good standing.
• Valid driver’s license and access to a vehicle.
• Experience delivering community/home care is an asset.
• Reliable, flexible, and willing to travel to patient residences.
• On-call coverage, including after hours, to ensure patient safety.

Responsibilities

• Use assessment tools to evaluate patients’ needs and monitor care plans.
• Execute and update care plans, coordinating with the care team.
• Engage patients through collaborative rapport-building and trust.
• Maintain and update professional records of patient encounters.
