Qualifications

• Bachelor of Science in Pharmacy required.
• Licensed Pharmacist in good standing in the province of responsibility.

Responsibilities

• Provide patient-centered care and counselling for new therapies.
• Ensure prescription safety and accuracy.
• Perform medication reviews, adherence support and disease state management as appropriate.
• Document care plans and follow up with patients/caregivers.
• Collaborate with colleagues to deliver health and wellness services across locations.
• Maintain knowledge of regulatory requirements and corporate programs.
