Job summary

Calgary Dream Centre is seeking a Community Case Manager to report to the Director of Community. The role covers case management, goal planning, client assessments, weekly check-ins, support and advocacy to help clients develop life skills.

You will work in a dynamic Community Programs environment, maintain detailed case files, and contribute to relapse prevention planning and reporting. Meetings and housing coordination with Property Management are part of the role.

Qualifications

• Post-secondary degree/diploma/certificate in Social Work or related discipline preferred.
• Minimum 3 years’ experience working in addiction services with marginalized populations.
• Valid Driver’s License with 2 million liability insurance.
• Strong communication skills, both written and verbal.
• Flexibility for after-hours work as required.

Responsibilities

• Mentor clients and monitor progress and interventions.
• Support housing needs and provide guidance while maintaining open communication.
• Maintain clean and safe property conditions for clients.
• Report maintenance needs and emergencies to Property Management.
• Track client progress and regularly review objectives.
• Maintain up-to-date case files with notes and relapse prevention plans.
