Job summary

Homewood Health Centre in Canada is seeking a Functional Recovery Support Case Manager to oversee a caseload of clients in treatment or post-assessment services, guiding them from assessment through to sustained recovery and return to work. You will develop tailored recovery plans, connect clients to appropriate supports, and monitor progress with the clinical team.

The role requires a Bachelor’s degree in Psychology, Social Work, or related field and 2–5 years in administration or case

Qualifications

• Bachelor’s degree in Psychology, Social Work, or related health discipline preferred (equivalent experience considered).
• 2–5 years of experience in administration or case management, with preference for case management background.
• Knowledge of case management principles and practices is an asset.
• Strong reading and interpretation of policy wording.

Responsibilities

• Manage a caseload of clients at the direction of a Clinical Lead, implementing and reviewing recovery plans.
• Connect participants to community resources, including health care, substance use treatment, mental health services, and return-to-work supports tailored to their goals.
• Collaborate with clients, employers, referral sources, and healthcare providers to support recovery and return-to-work outcomes.
• Monitor compliance, attendance, and progress, escalating concerns to the Clinical Lead when needed.
• Document case activity and maintain accurate, up-to-date clinical records and reports.
• Prepare progress reports, manage data and billing submissions, and participate in case and team meetings.
• Support departmental projects, provide coverage for colleagues as needed, and contribute to ad-hoc initiatives.
