Job summary

Southbridge Care Homes seeks an experienced RAI Clinical Coordinator to oversee MDS assessments, ensure accurate documentation, and support care teams in delivering high-quality resident care in Ontario LTC homes.

The role reports to the Director of Care and focuses on data analysis, process improvement, and staff training to optimize RAI content and PCC system usage.

Qualifications

  • 3-5 years' experience in Long Term Care, experience with MDS an asset.
  • Registered as a RPN under CNO.
  • Knowledge in nursing rehabilitation and working OT and PT.
  • Strong analytical and assessment skills.
  • Ability to lead training and education with Adult learners.
  • Knowledge of InterRAI suite of tools, MDS/RUG III patient classification systems.
  • Effective communication skills.

Responsibilities

  • Coordinate completion of MDS assessments on residents within established timelines.
  • Understand how the MDS assessment integrates with existing clinical tools and provides leadership in streamlining these processes.
  • Understand and provide critical analysis of data at different levels of aggregation (Resident, Home Health Zone, etc.).
  • Articulate for staff the strengths of MDS as a clinical assessment tool.
  • Articulate the financial implications of coding inconsistencies.
  • Act as key contact and liaison with: Health Zone/AH&W/CIHI/PCC, etc.
  • Develop facility specific material within the PCC program as required.
  • Respond to Health Zone on all submission errors.
  • Facilitate response and correction of submission errors with facility staff.
  • Coach to key facility personnel to build capacity for understanding RUG patient classification levels.
  • Develop and introduce processes, and systems to improve accuracy of RAI content.
  • Facilitate audits of RAI assessments for content accuracy.
  • Participates in strategic planning for program development/improvement.
  • Responsible for Clinical and Admin Setup role to support MDS within PCC software program.
  • Interprets reports generated by Health Zone/AH&W or CIHI and provides critical analysis for Home.
  • Understands the development of RAP's, quality indicators, and RUG case mix products.

Reports to: Director of Care

The RAI Clinical Coordinator supports residents by managing assessments, ensuring accurate documentation, and helping the care team provide the best possible environment and care.

Additional Requirements Include
  • A negative vulnerable sector check completed within 6 months of start date.
  • Completion of a 2 Step TB Skin test within the last 12 months of start date.
