Job summary

University Health Network seeks a PFCE Coordinator to advance patient, family and caregiver experience within the PMH portfolio. You will support project planning, data collection and reporting, and engage patient partners across committees.

Hybrid work environment with a focus on equity and quality improvement; prior oncology experience is an asset and collaboration across departments is essential.

Qualifications

• Minimum 2 years experience in patient experience, quality improvement, project coordination, or health services research.
• Experience with qualitative and quantitative data, dashboards, and reporting tools.
• Experience engaging directly with patients, caregivers and families in a healthcare setting.
• Excellent interpersonal, facilitation and organizational skills.
• Excellent analytical, problem recognition and resolution skills.
• Proficiency in Microsoft Office.

Responsibilities

• Coordinate end-to-end project management of multiple projects within a multi-disciplinary team environment.
• Assist with data collection, reporting and distribution – including data entry and data visualization (Excel).
• Liaise with team members and committees to ensure activities are coordinated.
• Prepare and maintain project plans, reports, presentations and executive summaries.
• Support Patient Family Caregiver Experience Committee and stakeholder engagement.
• Ensure patient feedback is representative and supports equity.

Position Summary:

The Patient, Family, CaregiverExperience(PFCE)Coordinator willbe an integral member of the PFCE portfoliowithin the Department of Strategy & Transformationat the Princess Margaret Cancer Centre, which aims to improvetheexperience of patients and familiesat The Princess Margaret by leveraging insights from patient feedbackwhile coordinating various initiatives and translating those insights into measurable improvement.They area highly organized individualwith background and experience in supporting healthcare-related projects.Primary responsibilities of this role includeeffective coordination and project planningas defined by portfolio priorities, implementation, documentation, monitoring and evaluation, support ofthe PatientFamilyCaregiverExperienceCommittee,and ongoing communication and strategic stakeholder/patient/caregiverengagement. This role will be in a hybrid work environment.

Duties:

• Project Management and Data Reporting
  • Coordinates end-to-end project management of multiple projects within a multi-disciplinary team environment.
  • Assistswithquantitative and qualitativeinformation collection,reporting, and distribution – including data entry and data visualization (Excel).
  • Liaises with all members of the team and committee/working groups to ensure project activities are coordinated.
  • Prepares andmaintainsproject plans,reports,presentations,executive summaries,and other keydocumentsrequiredto meet projectobjectives.
  • Demonstratesappropriate decision-makingwhen escalating and communicating issues during problem resolution/risk management.
  • Reviews the analysis, interpretation, and reporting of informationto ensure data quality/accuracy, asrequired.
• Patient Engagement
  • Review and synthesizequalitative and quantitative patient reported experience metrics (PREMS)and other related feedbackto support evidence-informed decisionsand priority-setting.
  • Provide coordination support for the Patient Family Caregiver Experience Committee, includingliaisingwith patient partners who are on the Committee.
  • Conduct patient follow up as necessarywithempathy, professionalism, compassion, and discretion.
  • Advise the teamtoensurepatient feedback is representative of thepopulations we serve to enhance equity.
  • Proactivelyidentifiesopportunities forquality and experienceimprovementinitiativesusing available data and insights.
