Our nurses, personal support workers, therapists, and other healthcare providers deliver care and services that offer clinical, personal, and social support in homes and communities across Nova Scotia and Ontario.

Region : Chatham – Kent

Description: Job Summary :

The Coordinator is responsible for supporting the Manager(s) and leading the daily operations within the framework of VON's philosophy, policies, procedures, standards, and quality management.

Key Responsibilities :
  1. Ensure the delivery of cost-effective client services and programs.
  2. Assess clients' service needs, monitor service delivery, and contribute to program development, evaluation, and promotion.
  3. Develop and implement program plans to meet departmental needs.
  4. Facilitate exercise classes and fall prevention education sessions for older adults at various community locations.
  5. Manage volunteers and staff, including orientation, training, and in-services, adhering to VON Quality Management standards.
  6. Engage regularly with clients and health partners to ensure programs meet community needs.
  7. Serve as a primary support contact among clients, health professionals, and community support teams.
  8. Track and report on program outcomes and team efforts to supervisors and stakeholders.
  9. Support financial accountability by managing program budgets.
  10. Prepare financial and statistical reports for VON and funder requirements.
  11. Conduct Disaster and Emergency Drills as per VON policies.
  12. Participate in multidisciplinary meetings as needed.
  13. Practice in accordance with VON's quality and risk management frameworks and occupational health and safety legislation.
  14. Complete health and safety training, including WHMIS, and apply this knowledge in the workplace.
  15. Report workplace incidents timely, per legislation and VON policies.
  16. Negotiate pricing with vendors and place service calls.
  17. Oversee daily office operations, source and order supplies, and support the CSS manager.
  18. Track expenses and oversee administrative team tasks.
  19. Collaborate with team members and community partners, maintaining accurate data and consolidating statistical information.
External and Internal Relationships :

Maintain timely communication with clients, community partners, and internal staff.

Education, Designations, and Experience :

Post-secondary diploma in health/social sciences or equivalent; experience in teaching senior fitness (in-person and virtual); 2-3 years in community/health care; 1-2 years in program planning (asset); experience in not-for-profit healthcare (asset).

Skill Requirements :

Commitment to confidentiality; strong knowledge of healthcare networks and community resources; excellent communication skills; ability to work independently and in teams; organizational and multitasking skills; problem-solving; proficiency in Windows and MS Office; attention to detail; high-quality customer service in a fast-paced environment.

Other :

Must have a personal vehicle, valid driver’s license, and proof of insurance. Criminal Records Check and possibly Vulnerable Sector Search/Child Abuse Registry Check required. French language skills are an asset. PPE may be required.

Salary : To be negotiated
