Job summary

Yee Hong Centre for Geriatric Care is hiring a Program Assistant for the Day Program (temporary full-time for 2 months, with extension possible). The role supports clients, plans activities, and ensures a safe, compassionate environment across Yee Hong sites in the Greater Toronto Area.

The position requires a diploma in Social Work or equivalent, experience in geriatric rehabilitation, and fluency in English and Chinese.

Qualifications

• Diploma in Social Work or Social Services, health sciences, activation, or equivalent.
• Experience in geriatric rehabilitation is essential.
• Excellent customer service skills.
• Ability to work independently and contribute effectively as a team player.
• Computer literacy in Microsoft Office Suite.
• Recent certification in CPR.
• Recent satisfactory Police Vulnerable Sector Screen result.
• Fluency in English and ability to converse in Chinese.
• Must be able to work across different Yee Hong sites as needed.
• Successful applicants must provide a recent satisfactory vulnerable sector police reference check and medical clearance certificate.

Responsibilities

• Under the overall direction of the Senior Coordinator/Program Coordinator, participates in the planning of appropriate activities for clients of the program
• Implements and ensures smooth operation of the activities in a safe, hygienic and supportive environment
• Provides timely and appropriate crisis intervention or behaviour management and ensures that the clients' physical, social and emotional needs are appropriately met
• Provides timely and appropriate support/information and referral/placement/outreach to clients/volunteers (as appropriate) to ensure that the clients/volunteers' social/ recreational/development needs are appropriately met
• Works closely with client and caregiver in ensuring appropriate design of care plan/program plan and ongoing monitoring of plan
• Ensures appropriate assessment, placement, matching and support for volunteers
• Provides effective guidance, training and support to the volunteers in the program
• Maintains up to date and accurate files, records and statistics on the program
• Maintains accurate record of fees and expenditures
• Complies with risk management policy for health and safety and infection control
• Keeps up-to-date with developments and best practices in the field
• Other duties as assigned

Salary Range: $22.52 - $26.49
