The Primary Care Nurse Practitioner (NP) provides comprehensive, evidence-based primary care to patients across the lifespan in a collaborative, interdisciplinary environment. The NP integrates clinical skills in assessment, diagnosis, treatment, health promotion, and disease prevention to improve patient and community health outcomes. This role includes providing care both in-clinic and through outreach initiatives to enhance access for underserved and vulnerable populations.
The Ottawa Nurse Practitioner-Led Clinic is dedicated to fostering a supportive environment for education and professional growth through the preceptorship of students. Responsibilities
- Provide holistic, patient-centered care including health assessments, diagnosis, ordering and interpreting diagnostic tests, prescribing medications, and therapeutic interventions within NP scope of practice.
- Manage acute and chronic health conditions in collaboration with the interdisciplinary team, specialists, and community partners.
- Promote health and prevent disease through patient education, counseling, and screening.
- Maintain accurate and timely electronic medical records and documentation in compliance with regulatory and clinic standards.
- Engage in quality improvement activities to enhance patient care, safety, and clinic operations.
- Participate in interdisciplinary team meetings, case conferences, and collaborative care planning.
- Advocate for patients, addressing barriers to care and connecting individuals with appropriate community resources and services.
- Provide mentorship, support, and education to learners and staff as appropriate.
- Stay current with best practices, clinical guidelines, and regulatory requirements for Nurse Practitioners in Ontario.
- Collect and submit statistics as required by Ontario Health to the Executive Director
- Has a sound understanding and works within the Ottawa NPLC Policies
- Design, deliver and evaluate health care programs for the Ottawa NPLC that are aligned with the strategic direction of the clinic and coordinated with all members of the health care team; that address current gaps in service, target population or population health

Qualifications
- Current certificate as a Nurse Practitioner-RN (EC) and Registered in good standing with the Ontario College of Nurses (CNO).
- Professional Liability protection (PLP) with CNPS.
- Bilingualism (English/French) and/or additional languages are considered strong assets.
- Satisfactory vulnerable sector check.
- Strong clinical skills
- Strong communication and interpersonal skills
- 3-5 years related or applicable experience in a community, hospital or primary health care setting (preferred but negotiable).
- Additional training or credentialing (e.g. Addictions Treatment, Mental Health Experience, Certified Diabetes Educator, Certified Asthma educator, etc.) is considered an asset.
- Experience managing chronic diseases (e.g., diabetes, hypertension, COPD), mental health concerns, and providing preventative care.
- Excellent clinical assessment, critical thinking, and communication skills.
- Proficiency with electronic medical records (EMR) systems.
- Commitment to working collaboratively in an interdisciplinary team environment.
- Experience in program design, delivery and evaluation an asset
- Current BCLS/Naloxone Training
Remuneration
- Annual Salary range: TBD
