Position Summary

In accordance with established mission, vision, values and strategic priorities of the Northern Health and reporting to the Pharmacy Operations Lead, Primary and Long-Term Care, this position provides clinical pharmacy services to patients of the primary care network. As a member of an interprofessional team this position works directly with patients to identify and resolve actual and potential drug therapy problems through provision of comprehensive medication management.

Shift Rotation/Hours of work

Days, 08:30 to 16:30 (Rotating)

Location

Northern Haida Gwaii Hospital and Health Centre provides medical service to Old Masset, Port Clements and surrounding areas. The hospital and health centre has 4 Acute Care beds, 1 Palliative Care bed, 4 Long-term Care beds and a 4 unit assisted living complex. Some services provided at the hospital include:

  • Emergency
  • Laboratory
  • Diagnostic Imaging
  • Pharmacy
  • Chronic Disease Management
  • Physiotherapy
  • Speech and Language therapy
  • Mental Health and Substance Use service
  • Home and Community Care and Home Support services
  • Visiting specialists in Pediatrics, Orthopaedics, ENT, Nephrology, Endocrinology, Respirology, Rheumatology, Urology and Ophthalmology
  • Video conferencing capabilities with specialists or physicians in other areas

Public Health provides many services to the community, including:

  • Harm reduction services
  • TB testing
  • Prenatal services with pregnancy registry
  • Kindergarten screening and school immunizations
  • Birth control, pap tests, STI testing
About Masset

Masset is situated along the shoreline of Masset Sound on Haida Gwaii and is the largest village on the island. The community serves a larger rural area with a population of approximately 1,000 local residents and surrounding areas of about 2,300 including Tow Hill and Old Masset. Qualifications
  • Current registration with the College of Pharmacists of British Columbia and a minimum 2 years recent, related patient care experience, ideally in a team based primary care setting or equivalent.
  • A valid Class 5 drivers' licence and access to a personal vehicle for business related purposes.

  • Ability to complete initial and ongoing patient assessments (clinical and diagnostic reasoning) through appropriate/prescribed technical, therapeutic, safety type interventions.
  • Ability to teach patients and others both one-on-one and in groups.
  • Integrates best practice and current research evidence to support professional practice decisions and actions.
  • Ability to communicate effectively with patients, families, the public, medical staff and members of the interdisciplinary team using verbal, written, computer communication means. Ability to effectively apply conflict resolution skills.
  • Ability to integrate and evaluate data to problem-solve and make decisions effectively; applies the problem-solving process with critical thinking and a systems approach.
  • Promotes patient-centred care with sensitivity to diverse cultures and preferences, patient advocacy and social justice concerns.
  • Ability to organize work, set objectives and establish priorities; manages time and resources; collaborates across disciplines.
  • Promotes staff morale, engagement, and empowerment; supports change and professional development of self and others.
  • Fosters teamwork and a commitment to excellence in patient care.
  • Demonstrated computer skills including EHR, Windows-based programs, email and word processing; ability to operate related equipment in the practice area.
  • Physical ability to perform the duties of the position.
