Job Description

In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority, patient safety is a priority and a shared responsibility. Continuous quality and safety improvements are inherent in this position.

Situated within identified primary care network locales, this position provides clinical pharmacy services to patients in the primary care network. As a member of an inter-professional team, the pharmacist works directly with patients to identify and resolve drug therapy problems through comprehensive medication management.

Travel may be required, and transportation arrangements must meet operational requirements, potentially involving the use of a personal vehicle.

Qualifications:

• Current registration with the College of Pharmacists of British Columbia.
• Minimum 2 years' recent patient care experience, preferably in a team-based primary care setting or equivalent.
• Valid Class 5 driver's license.
