As a Clinical Pharmacist, you play a vital role in ensuring continuity of care for clients in long-term care, supportive living, and other congregate living communities. Collaborating with onsite physicians, nursing staff, and other healthcare providers, you are responsible for maintaining high standards of patient care. In your role, your daily responsibilities will include:

1. Conducting medication reconciliation for all CareRx clients pending admission to long-term care, retirement living, and other facilities, aiming to improve patient outcomes.
2. Providing comprehensive pharmaceutical care in collaboration with the interdisciplinary team and offering clinical recommendations.
3. Working with the healthcare team to develop strategies that optimize resident care during transitions and improve outcomes.
4. Producing quality metrics related to clients and medications during transitions and supporting departmental initiatives to ensure quality patient care in line with CareRx and Department policies.
5. Liaising between the dispensing pharmacy, management, and staff to ensure optimal pharmacy services are delivered to each facility.

Note: This is a salaried position. While hours are typically 9 AM to 5 PM, they may vary based on organizational needs.
