Home Health visits without the travel! This position will cover Senior Living Communities (ALF/ILF) only, providing home health services.

$2,500 SIGN ON BONUS FOR FULL TIME

BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT for a per diem, part-time, or full-time opportunity performing home health visits for our senior living office. This office services adult and geriatric clients on a per-visit basis in territories throughout the Harrisonburg/Staunton, VA area.

As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to remain at home.

Responsibilities for a PT - Physical Therapist include:

1. Make home visits to clients in designated geographic territories.
2. Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns and appearance, muscle strength and endurance, balance testing, and safety assessments.
3. Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
4. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
5. Assess and revise the care plan continually, collaborating with nursing in a multidisciplinary care team as appropriate.
6. Educate and instruct clients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independence.
7. Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

1. A current VA Physical Therapist - PT license.
2. Graduation from an accredited program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Solid computer skills; prior experience with electronic medical records (EMR) preferred.
