Job summary

Carefor Health & Community Services is seeking a Personal/Home Support Worker to provide in-home personal support and homemaking services in Eastern Ontario. The role involves night shifts (2300-0700) for a temporary 3-month term with potential extension, at 0.4 FTE.

Responsibilities include daily living activities, meal prep, light cleaning, and documentation of care plans. Requires relevant PSW certification and clear checks; HOOPP pension and benefits are offered.

The Personal/Home Support Worker provides personal support and homemaking services to individuals and families in the home within the framework of Carefor policies, procedures, standards and quality & risk management. Collaborates with all members of the health care team to meeting the needs of individuals and attain an optimal level of independence and well-being.

Temporary 3 months with the possibility of extension, PT 0.4

Hours of work 2300-0700; Week 1- Friday; Week 2- Friday, Saturday, Sunday

• Assist in the activities of daily living including eating, dressing, transferring, grooming, and bathing
• Provide laundering of clothes and linens
• Provide light cleaning
• Provide respite care
• Provide light meal preparation
• Provide cognitive, social, and compassionate support
• Document reports for clients on their plan of care and any changes in the behaviour of daily routines
• Other related duties and tasks as assigned by the Supervisor in compliance with the Occupational Health and Safety legislation, regulations, and Carefor policies and procedures.
• Ontario Certified Personal Support Worker Certificate and/or National Association of Career Colleges (NAC) Certificate, or Certificate in Home Support Worker (Level II, III), or Developmental Service Worker program, Health Care Aide, Foreign-Trained BScN or Doctor, Paramedic Graduate, BScN Nursing Student, or second year RPN student.
• Clear Vulnerable Sector check (obtained within previous six months)
• Up to date immunizations for diphtheria, tetanus, polio, rubella, and hepatitis
• Negative tuberculosis test
• Experience with Palliative Care is considered an asset
• First Aid CPR is considered an asset
