Job summary

Carefor is seeking a dedicated Personal/Home Support Worker to provide in-home care across Eastern Ontario. The role collaborates with the health care team to support clients in achieving independence and wellbeing, with weekday daytime hours (8:00 a.m. to 4:30 p.m.).

You will assist with daily living activities, light housekeeping, meal prep, respite care, and personal support, while documenting changes to care plans and ensuring safety per Carefor policies.

Qualifications

• Ontario Certified Personal Support Worker Certificate and/or NAC/NAS or equivalent health care aide credential.
• Clear Vulnerable Sector check completed within the last six months.
• Up-to-date immunizations for diphtheria, tetanus, polio, rubella, and hepatitis.
• Negative tuberculosis test.
• Palliative care experience is an asset.
• First Aid CPR is an asset.
• Vehicle is required.

Responsibilities

• Provide laundering of clothes and linens
• Provide light cleaning

• Provide light meal preparation

Overview

The Personal/Home Support Worker provides personal support and homemaking services to individuals and families in the home within the framework of Carefor policies, procedures, standards and quality & risk management. Collaborates with all members of the health care team to meeting the needs of individuals and attain an optimal level of independence and well-being.

Please note that this role is Monday to Friday, from 8:00 a.m. to 4:30 p.m. There are no weekend or holiday shifts—only daytime shifts.

• Ontario Certified Personal Support Worker Certificate and/or National Association of Career Colleges (NAC) Certificate, or Certificate in Home Support Worker (Level II, III), or Developmental Service Worker program, Health Care Aide, Foreign-Trained BScN or Doctor, Paramedic Graduate, BScN Nursing Student, or second year RPN student.
• Clear Vulnerable Sector check (obtained within previous six months)
• Up to date immunizations for diphtheria, tetanus, polio, rubella, and hepatitis
• Negative tuberculosis test
• Experience with Palliative Care is considered an asset
• First Aid CPR is considered an asset
• Vehicle is required
