Job summary

SE Health in Ontario seeks a registered nurse to deliver comprehensive home and community care. You will assess clients, develop care plans, and implement care with evidence-based practices while collaborating with the care team.

The role requires travel in Guelph and surrounding areas, availability for days, evenings, nights, and alternating weekends, and active CPR and nursing credentials. SE Health emphasizes growth, benefits, and a positive work‑life balance.

Qualifications

• Current Certificate of Competence with the College of Nurses of Ontario.
• Availability to work days, evenings, nights and every other weekend.
• Willingness to attain SE Health certifications and maintain immunization records.
• Updated immunization records and clear background checks required at hire.
• Current CPR certificate required.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence and client needs.
• Act as primary provider for a client caseload when designated, leading care with the team.
• Assist clients through transitions by connecting with community support services.

Responsibilities:

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements:

• Current Certificate of Competence with the College of Nurses of Ontario.
• Availability to work days, evenings, nights and every other weekend.
• Able to travel around Guelph, Elora, Rockwood, Fergusand surrounding areas.
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required SE Health certifications
• A current CPR certificate is required.
• This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
