Job summary

SE Health in Ontario seeks Registered Nurses for flexible, full-time assignments in Guelph and surrounding areas. You will assess clients, develop care plans, and collaborate with the care team to deliver high-quality nursing care. Travel among Guelph, Elora, Rockwood, Fergus and nearby communities is required.

Qualifications

• Current Certificate of Competence with the College of Nurses of Ontario.
• Availability to work days, evenings, nights and every other weekend.
• Able to travel around Guelph, Elora, Rockwood, Fergus and surrounding areas.
• Updated immunization records, clear criminal background check, and Vulnerable sector check on hire.
• Willingness to attain SE Health certifications
• Current CPR certificate is required.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care.
• Act as primary provider for a client caseload when assigned, coordinating with the care team.
• Assist clients through transitions by connecting with community support services.

Responsibilities:

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements:

• Current Certificate of Competence with the College of Nurses of Ontario.
• Availability to work days, evenings, nights and every other weekend.
• Able to travel around Guelph, Elora, Rockwood, Fergusand surrounding areas.
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required SE Health certifications
• A current CPR certificate is required.
• This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
