Job summary

SE Health is hiring registered or registered practical nurses to join a dedicated team in Amherstburg, Ontario. The role involves conducting comprehensive client assessments, developing and evaluating care plans, and delivering evidence-based nursing care to diverse clients.

Successful candidates will hold an active Certificate of Competence with the College of Nurses of Ontario and have a valid CPR certificate.

Qualifications

• Current nursing licensure with the College of Nurses of Ontario.
• CPR certification is required.
• Immunization records and background checks are required on hire.
• Willingness to obtain SE Health certifications.

Responsibilities

• Perform comprehensive client assessments and develop care plans.
• Act as primary provider for client caseload when assigned; coordinate with the care team.
• Assist clients in transitions and connect them with community services.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements

• Current Certificate of Competence with the College of Nurses of Ontario.
• Availability to work weekdays and minimum of 16hours on weekends (Saturday and Sunday).
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required SE Health certifications
• A current CPR certificate is required.
• This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
• Required to travel to Amherstburg and surrounding areas.
