Job summary

SE Health is hiring in Hamilton, Ontario for a replacement, full-time nursing role. You will complete comprehensive client assessments, develop and evaluate care plans, and collaborate with the care team to ensure high-quality care tailored to each client.

Required are a Certificate of Competence with the College of Nurses of Ontario and a current CPR certificate, plus immunization records and background checks.

Qualifications

• Current Certificate of Competence with the College of Nurses of Ontario.
• Availability to work days, evenings, nights and every other weekend for 12 hour shift.
• Updated immunization records and clear background checks are required on hire.
• Willingness to attain SE Health certifications.
• A CPR certificate is required.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence and client needs.
• Act as primary provider for a client caseload when designated and coordinate with the care team.
• Support clients through care transitions by connecting them with local community services.

Responsibilities:

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements:

• Current Certificate of Competence with the College of Nurses of Ontario.
• Availability to work days, evenings, nights and every other weekend for 12 hour shift.
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required SE Health certifications
• A current CPR certificate is required.
• This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
