Job summary

SE Health is hiring for part-time community shift roles in Central West Region (Brampton/Caledon/Etobicoke/Woodbridge). The position requires day/evening/night availability, and every-other-weekend work. A current nursing certificate and CPR are required, with opportunities to grow and earn a sign-on bonus.

• Current Certificate of Competence with the College of Nurses of Ontario.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices.
• Be primary provider for a client caseload and coordinate with the care team.
• Support clients through transitions by linking with community services.

Responsibilities:

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements:

• Current Certificate of Competence with the College of Nurses of Ontario.
• Availability to work days, evenings, nights and every other weekend.
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required SE Health certifications
• A current CPR certificate is required.
• This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
