Saint Elizabeth is hiring a Full-Time Contract Shift Registered Nurse (RN) / Registered Practical Nurse (RPN) to work with pediatric clients in the community and school settings, based in Guelph, Ontario. The role involves providing holistic, evidence-informed care to children and families across home and community settings.

You will assess, plan, implement and evaluate care, acting as a primary provider for a client caseload and collaborating with a multidisciplinary team.

Qualifications

• Current registration with the College of Nurses of Ontario.
• CPR certification required.
• Willingness to travel and valid driver’s license.
• Experience in pediatric community care is an asset.

Responsibilities

• Complete comprehensive client assessments and develop care plans.
• Serve as primary care lead for designated client caseload.
• Coordinate care transitions and connect clients with community supports.
