Job Title: Patient Service Coordinator (Contract)
The contract Patient Service Coordinator, under the direction of the Patient Services Supervisor, Specialty Nursing Services, is responsible for supporting patients with their care coordination needs, including but not limited to the professional and timely scheduling of patient services within the Specialty Nursing Network. The Patient Service Coordinator is also responsible for scheduling our field employees and ensuring that the key performance indicators for scheduling are met.
This role is a 12-month Contract position.
DUTIES AND RESPONSIBILITIES
- Handle and document patient inquiries regarding requests for service.
- Report and document Adverse Events as per Pharmacovigilance requirements.
- Assess patient service needs in consultation with the Supervisors.
- Schedule licensed employees for care and service assignments, ensuring an appropriate match between patients' needs and employees’ qualifications, skills, and abilities.
- Promote consistency of nursing assignments and coordination of services.
- Notify patients and field employees regarding initial and ongoing schedules.
- Complete data entry and maintain accurate current scheduling documentation.
- Be available for On-Call shifts, including mornings, evenings, weekends, or holidays.
- Handle and document patient concerns and complaints promptly and escalate as appropriate.
- Meet required scheduling key performance indicators.
- Ensure scheduling is efficient and patient-centric.
- Participate in ongoing internal and external continuing education activities.
- Adhere to Bayshore Policies and Procedures.
- Participate in quality activities and continuous improvement initiatives in line with the company's Quality Management System.
- Participate in proactive Health & Safety activities during all duties.
- Maintain confidentiality of patient and corporate information, discussing it only with authorized personnel.
- Maintain relevant knowledge of ICH, GCP, and Health Canada GVP guidelines, with updates provided regularly.
- Participate in processes related to regulatory and contractual reporting requirements.
- Perform other tasks as assigned.
Job Qualifications
Education
- Secondary School Diploma is required.
- Completion of a recognized program such as Licensed Registered Practice Nurse or Medical Terminology course is preferred.
Experience
- At least two years of related customer service experience, preferably in healthcare.
Other Skills and Abilities
- Fluency in English, both written and verbal.
- Bilingual in English and French, with strong communication skills, especially for roles supporting patients in Quebec and Northern New Brunswick.
- Exceptional organizational and interpersonal skills.
- Ability to work independently and in a team.
- Ability to meet deadlines and a commitment to continual learning.
- Proficiency in keyboarding and Windows scheduling software.
- Ability to operate standard office equipment.
- Internal candidates should have a proven track record with patients and staff, demonstrating initiative. They must complete a three-month probationary period specific to this role.
