Job summary

Family Service Thames Valley (FSTV) is a not-for-profit agency that supports Passport funding, developmental services, and counseling to strengthen individuals and families in Thames Valley. The Passport Coordinator/Case Manager role handles individualized plans, referrals, intakes, budgets and receipts, with on-site Monday–Friday work.

Reporting to the Passport Program Manager and the Director of the Community Integration Program, the position requires education in human services, 5+ years in

Qualifications

• Post-secondary education in human services or equivalent experience.
• Minimum 5 years in the development sector; case management is an asset.
• Excellent interpersonal and written/verbal communication skills.
• Access to a vehicle and willingness to travel.

Responsibilities

• Coordinate Passport plan components and liaise with referral agencies.
• Complete intakes, build client rapport, manage budgets and receipts.
• Administer client profiles and reconcile finances accurately.
• Advocate for clients and promote inclusivity and community engagement.
• Collaborate with partners and stay informed on local resources.
