Job summary

ParaMed is seeking a Palliative Registered Nurse (RN) to provide scheduled home visits in Kitchener and Cambridge. Both full-time and part-time positions are available, with day and evening shifts and every third weekend.

You will travel within the region, assess patients, and implement care plans to keep patients comfortable at home. The role requires an active RN license with CNO in good standing, driving eligibility, and mandatory palliative care experience.

Qualifications

  • Registered nurse with CNO in good standing and current license.
  • Mandatory palliative care experience in community settings.
  • Post-graduate education in palliative care preferred.
  • Experience delivering home/community-based care is an asset.

Responsibilities

  • Assess patients using appropriate tools to identify needs.
  • Execute and monitor care plans; update records accordingly.
  • Engage patients and families with rapport and collaboration.

You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.Full-Time and Part-Time opportunities available. 8am to 4pm or 2pm to 10pm, includes every third weekend.
