Job summary

ParaMed in Canada is seeking a Palliative Registered Practical Nurse (RPN) to provide community-based nursing care across Kitchener and Cambridge. You will perform scheduled visits, deliver ethical care and monitor care plans within your scope, while building trusted relationships with patients and families.

The role offers full-time and part-time opportunities with day or evening shifts, and requires willingness to travel to patient residences. A driver’s license is required.

Qualifications

• Registration with the CNO as a Registered Practical Nurse in good standing.
• Palliative care experience is mandatory.
• Post-graduate education in Palliative care is required.
• Valid driver’s license and access to a vehicle is required.
• Experience in delivering community care is an asset.
• Willingness to travel to and from patient residences within a designated area.
• Required on-call coverage, including after hours to ensure patient safety

Responsibilities

• Participate in scheduled visits in the community to look after patient needs within your scope of practice.
• Provide ethical and safe nursing care.
• Use appropriate assessment tools/techniques to assess patients’ individual needs.
• Execute and continuously monitor care plans and adjust as required.
• Engage patients using a collaborative approach by building rapport and trust.
• Maintain and update professional patient records related to patient encounters and plans.

In this community nursing role, you as a Palliative Registered Practical Nurse (RPN) will have scheduled visits within the community to care for patients in various settings with the support of RNs for more complex patients. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible. Full-Time and Part-Time opportunities available. 8am to 4pm or 2pm to 10pm, includes every third weekend. Must be willing to travel anywhere within Kitchener and Cambridge.
