Job summary

SE Health in Ontario is seeking an Occupational Therapist to deliver client-centered care in home and community settings. You’ll conduct assessments, plan goals, and implement individualized treatment within a collaborative rehab team to support independence and recovery.

This hybrid role combines remote coordination with hands-on care across acute/rehab or community settings. We value strong communication, teamwork, and the ability to work independently.

Qualifications

  • Graduate of an accredited OT program.
  • Member in good standing of the College of Occupational Therapists of Ontario (COTO).
  • Experience in acute/rehab or community rehabilitation programs preferred.
  • Ability to work independently and collaboratively within an interdisciplinary rehabilitation team.
  • Knowledge of community resources is an asset.

Requirements
  • A graduate of an accredited program in Occupational Therapy and are a member in good standing of the College of Occupational Therapists of Ontario (COTO registration)
  • Recent experience in managing patients inacute/rehab or community-based rehabilitation programs is preferred
  • Demonstrated ability to work independently and collaboratively within an interdisciplinary rehabilitation team while supporting positive client outcomes
  • Sound knowledge of community resources is an asset
  • Effective interpersonal, organizational and communication skills
  • A positive work and attendance record is essential
  • Ability to adapt to new computer applications

This role follows a hybrid work approach where you are able to work remotely and with the clients in their acute/rehab or community-based rehabilitation setting
