Job summary

Carecor Health Services Ltd. invites qualified Occupational Therapists to join our team serving Waterloo region. You will assess clients, design individualized treatment plans, and support daily living independence, including home safety and discharge planning.

Expect collaboration with a multidisciplinary team and opportunities for flexible shifts within a busy on-site setting. Casual and part-time options are available, with shifts days and evenings.

Qualifications

• Masters degree in Occupational Therapy or equivalent.
• Registered in good standing with the College of Occupational Therapists of Ontario (COTO).
• ADP considered an asset.

Responsibilities

• Conduct assessments to determine clients' functional abilities and rehabilitation needs.
• Develop and implement individualized treatment plans.
• Support clients in improving independence with activities of daily living.
• Facilitate smooth transitions from hospital to home by conducting home safety assessments and discharge planning.
• Recommend adaptive equipment, mobility aids, and environmental modifications.
• Educate clients, caregivers, and families on strategies to support recovery and independence.
• Collaborate with physicians, nurses, physiotherapists, social workers, and other healthcare professionals to ensure coordinated care.
• Monitor client progress and adjust treatment plans as needed.
