CBI Home Health is seeking an Occupational Therapist to serve North Wellington County (Mount Forest, Arthur, Clifford, Shelburne). This flexible role offers a mix of home visits and community care, with a patient‑centered approach and collaboration with an interdisciplinary team to help clients live safely and independently at home.

You will assess, implement OT interventions, provide ADL/IADL supports, make home modifications, and use digital health tools for care, documentation, and reporting.

Qualifikationen

• Registered with the College of Occupational Therapists of Ontario in good standing.
• Valid driver’s license and ability to travel within the community.
• Patient-centered care with strong communication and empathy.
• Ability to work effectively in both team settings and independently.

• Provide OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications.
• Collaborate with an interdisciplinary care team.
• Support restorative approaches with health coaching and self-management.
