Carecor Health Services is seeking dedicated Occupational Therapists to join our team in Waterloo, ON and surrounding areas. You will assess clients’ physical, cognitive, and functional abilities, and develop individualized plans to support independence and daily living goals.

You will guide transitions from hospital to home, assess home environments, and recommend modifications and assistive devices. Collaboration with a care team helps reduce re-hospitalization and promotes long-term

Formación

• Masters degree in Occupational Therapy or equivalent.
• Registered in good standing with the College of Occupational Therapists of Ontario (COTO).
• ADP considered an asset.

Responsabilidades

• Conduct assessments of clients' functional abilities and rehabilitation needs.
• Develop and implement individualized treatment plans.
• Support clients to improve independence in activities of daily living.
• Facilitate transitions from hospital to home and conduct home safety assessments.
• Recommend adaptive equipment, mobility aids, and environmental modifications.
• Educate clients, caregivers, and families on strategies for recovery and independence.
• Collaborate with physicians, nurses, physiotherapists, social workers and other health professionals.
• Monitor progress and adjust treatment plans as needed.

Type of Work: Days, Evenings
Hourly Rate: $50.28 to 54.63 per visit + mileage

Carecor Health Services is looking for dedicated and compassionate Occupational Therapists to join our amazing team.

As an Occupational Therapist, you will work closely with clients to assess their physical, cognitive, and functional abilities, helping them achieve greater independence and quality of life. Through comprehensive evaluations and individualized care plans, you will recommend therapeutic interventions, adaptive strategies, and equipment to support clients in meeting their daily living goals.

A key component of this role is supporting clients through their transition from hospital to home. You will collaborate with interdisciplinary healthcare teams, clients, and families to ensure a safe and successful discharge process. This includes assessing home environments, identifying potential barriers to independence, recommending modifications or assistive devices, and providing education to help clients navigate daily activities confidently in their home setting. Your expertise will help reduce the risk of re-hospitalization while promoting recovery, safety, and long-term well-being.

• Must be available for a variety of shifts; days, evenings, weekdays, and weekends.

Key Responsibilities

• Monitor client progress and adjust treatment plans as needed.

Job Qualifications

• Masters degree in Occupational Therapy or equivalent
• Registered in good standing with the College of Occupational Therapists of Ontario (COTO)

Requirements

• Proof of vaccination against COVID-19 (minimum 2 doses), Hepatitis, Flu, etc.
• Willing to undergo a Police Background Check and/or Vulnerable Sector Search as a condition of employment, with 'clear' results
• Valid drivers license and access to reliable transportation
