Job summary

CBI Home Health is seeking an Occupational Therapist to serve clients in the Cambridge Region, including Guelph, ON. The role supports home-based care with flexible scheduling and visits across the community.

The position offers a sign-on bonus and opportunities to grow within a leading Canadian home health provider. You will assess and implement OT interventions, collaborate with an interdisciplinary team, and promote holistic, restorative care using digital tools for documentation and care

Qualifications

• Must be a registered member in good standing with the College of Occupational Therapists of Ontario.
• Must have a valid driver’s license and be able to travel within the community.
• Demonstrates client- and family-centered care with empathy.
• Exhibits strong critical thinking and judgment in client care decisions.
• Comfortable collaborating with teams and also working independently.
• Committed to enabling clients to remain at home when aligned with goals.

Responsibilities

• Assessment & Implementation: Provide OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications.
• Team Collaboration: Use a collaborative, interdisciplinary approach to care and services.
• Holistic Care: Support a restorative approach to health, offering interventions, health coaching, and self-management support.
• Digital Health: Utilize our digital platforms for care delivery, collaboration, documentation, and intervention.

Salary Range: The salary range for this position is $57.44 – $67.39 per visit
