Job summary

AgeCare is seeking a Nurse Practitioner to collaboratively deliver resident care at AgeCare Queen’s Garden in Hamilton, ON. You will work with physicians and the interdisciplinary team to assess, diagnose, and manage acute and chronic conditions while following regulatory standards.

Responsibilities include care conferences, care leadership for the LTC team, and engaging with residents, families, and SDMs. Prior experience in geriatric or long-term care and relevant certifications are valued.

Qualifications

• Registration as a Registered Nurse in the Extended Class (PHC or Adult) in Ontario.
• Experience or leadership in geriatric, rehab, or long-term care settings.
• Certifications in gerontology, mental health, behavioral support, or palliative care are assets.

Responsibilities

• Collaborate with MD, DOC, and interdisciplinary team to deliver resident care.
• Assess, diagnose, and manage acute/chronic conditions; prescribe treatment and follow up.
• Advocate for residents and engage SDMs about care planning.
• Lead and participate in resident care conferences and educational activities.
• Provide clinical leadership for LTC team including discharge planning and palliative care.
• Identify trends in care and determine priorities for team education.
