Job summary

AgeCare is seeking a Registered Practical Nurse (RPN) to provide professional nursing care for residents in a long-term care setting on site in Ontario. The RPN will work in collaboration with the Registered Nurse, oversee Personal Support Workers, and ensure adherence to nursing standards and Health Act regulations.

You will liaise with physicians and the health care team, monitor resident status, and participate in shift reports to keep everyone informed.

Qualifications

• Geriatric nursing expertise and rehabilitation knowledge.
• Ability to oversee Personal Support Workers and coordinate care.
• Familiarity with statutory requirements and patient safety standards.

Responsibilities

• Collaborate with the RN to implement AgeCare policies on the unit.
• Liaise with physicians and health care partners and inform of status changes.
• Coordinate care to meet residents' needs with the health care team.
• Provide direction to Personal Support Workers on patient care.
• Participate in shift reports ensuring team is updated on resident status.
