Job summary

Pinecrest-Queensway Community Health Centre is seeking a Nurse Practitioner for the West Ottawa HART Hub. This regular part-time role provides integrated withdrawal management, addiction medicine, episodic primary health care and mental health assessment for clients 16+.

You will work within a trauma-informed, culturally safe framework, assessing, diagnosing, prescribing and following up within NP scope. Join an interdisciplinary team, coordinate care, support discharge planning and contribute

Qualifications

• NP must hold Baccalaureate Degree in Nursing and current registration in the Extended Class as NP with the College of Nurses of Ontario.
• Experience or willingness to learn addiction medicine, withdrawal management, primary care, mental health, and care for complex health and social needs.
• Proficiency with electronic health records and related software; bilingualism preferred.

Responsibilities

• Provide client-centred, trauma-informed care within NP scope of practice.
• Assess, diagnose, treat, prescribe and monitor care for substance use, physical health and mental health concerns.
• Deliver withdrawal management, addiction medicine, episodic primary health care and mental health services.
• Identify health risks, develop care plans and support clients in health and recovery goals.
• Prioritize care based on safety, clinical risk and continuity of care.
• Facilitate referrals, consultations and transitions to primary care, psychiatry and community supports.
• Collaborate with clients, caregivers and interdisciplinary team to ensure coordinated care.
• Maintain clinical documentation and participate in quality assurance initiatives.
