Job summary

Pinecrest-Queensway Community Health Centre is seeking a Nurse Practitioner for the West Ottawa HART Hub to deliver integrated withdrawal management, addiction medicine support, episodic primary care and mental health assessment to clients aged 16 and older.

Working within a trauma-informed, client-centred and harm-reduction framework, you will assess, diagnose, prescribe and coordinate care, with strong collaboration across interdisciplinary teams and community partners.

Qualifications

• Current registration as a Nurse Practitioner in Ontario.
• Experience or willingness to learn addiction medicine, withdrawal management, primary care, mental health.
• Knowledge of evidence-informed substance use treatment and harm reduction.

Responsibilities

• Provide client-centred, trauma-informed, culturally safe care.
• Assess, diagnose, treat, prescribe and monitor substance use, physical and mental health concerns.
• Deliver withdrawal management and episodic primary health care.
• Coordinate care and refer to primary care, specialists and community supports.

Education

Baccalaureate Degree in Nursing

Registered as Nurse Practitioner (Extended Class)

Bilingualism preferred
