Pinecrest-Queensway Community Health Centre (PQCHC) seeks a Nurse Practitioner to join its Integrated Health Services in Ottawa. The NP will work in primary care, managing a panel of clients with autonomy and access to physician consultation when needed.

Qualifications

• Master’s degree in Nursing and current registration in good standing with the College of Nurses of Ontario (CNO) in the Extended Class (NP-PHC or NP-Adult).
• Minimum of two (2) years of experience practicing as a Nurse Practitioner, ideally in a primary care or community health setting.
• Linguistic profile: Bilingual English/French is an asset.

Responsibilities

• Works independently as the Most Responsible Provider for a dedicated panel of clients with physician consultation as appropriate.
• Diagnose, treat, and manage acute and chronic conditions across the lifespan.
• Collaborate with physicians, registered nurses, allied health professionals, and social services staff to deliver team-based primary care.
• Conduct comprehensive health assessments, develop and review care plans, and provide direct clinical services.
• Order and interpret diagnostic investigations, prescribe medications, and coordinate referrals to specialists and community resources.
• Educate clients on disease prevention, health promotion, and chronic disease management.
• Maintain accurate documentation in the electronic medical record.
